DESPRENDIMIENTO DE RETINA REGMATÓGENO. EVALUACIÓN DE LOS RESULTADOS ANATÓMICOS Y FUNCIONALES DE ACUERDO AL PROCEDIMIENTO QUIRURGICO REALIZADO

Resumen


Objetivo: Determinar el éxito anatómico, funcional y quirúrgico de las cirugías de DRR y su correlación con el procedimiento quirúrgico y las características del desprendimiento. Métodos: Estudio descriptivo, retrospectivo no randomizado. Muestra de 116 ojos con DRR intervenidos quirúrgicamente por VVPP o cirugía combinada (cerclaje escleral más VVPP o cerclaje escleral más exodrenaje). Resultados: La edad promedio fue de 56 ± 14 años, siendo el 69.8 % masculinos y el 54.3% ojos derecho. El tiempo promedio de seguimiento fue de 19.9 meses (rango, 3- 79 meses). 57.8% eran fáquicos y 42.2% pseudofáquicos. El número promedio de roturas retinianas fue de 2.01, y el de cuadrantes afectados fue de 2.27. 80.1% DRR MAC-OFF. 11% PVR. La AVfinal fue significativamente mejor en los ojos con menor tiempo de enfermedad (p=0,002), MAC-ON (p=0,002), menor número de cuadrantes afectados (p=0,002), así como el éxito anatómico y funcional (p=0,029), y quirúrgico primario (p=0,011). AVfinal sin diferencia entre grupos de cirugía (p=0,092). El éxito anatómico fue de 29 de 30 ojos (96,7%) en el grupo de VVPP y de 85 de 85 (100%) ojos en el grupo de CE/VVPP (p=0,402), con un éxito quirúrgico primario en 22 de 30 (75,9%) vs 35 de 85 (41,2%) (p=0,003). No hubo diferencia significativa en la AVfinal fue de 0,30 LogMAR en VVPP y 0,40 en cerclaje más VVPP (p=0,092). Conclusiones: La VVPP y el CE/VVPP tienen similar eficacia en la reparación del DRR. El tiempo de enfermedad, número de cuadrantes y compromiso macular influyen en la AVfinal.
